Below you will find the forms you will need for the start of the 2020-2021 school year. All of these forms can be accessed and/or completed online through our school webpage or weCare@school. Please review the table below for required forms as well as information about accessing them online.

Health Commitment Form	Complete this form before your child returns to the school building. This document confirms that a parent, caregiver, or guardian will complete a daily health screening and will keep their students home should they become ill.
